Drs. VanderLaan and Eisenga work together with your doctor to provide expert NF diagnosis and follow-up care. It is preferred that
patients have a primary care provider (personal physician) before attending clinic. For children this could be a pediatrician or
family practitioner, for adults an internist or family practitioner. The NF clinic will report the results of your exam and follow-up
recommendations to your primary care provider. If you have no primary care doctor, you may call the Spectrum Health Physician Finder
Service at 616-391-5999 to help locate one in your area.
MAKING AN APPOINTMENT: Before making an appointment, discuss with your doctor your interest in attending NF Clinic. HMOs,
PPOs, most Medicaid plans and many other insurance plans will pay only if your physician approves the visit in advance. When you
call the clinic, be prepared to give names and addresses of doctors, hospitals and/or clinics that have your medical records. Please
allow at least two hours for the first visit. Return visits usually run under one hour.
When you call for an appointment you will probably get a recording. Please leave your name, phone number and the best time to reach
